Outlier Health and Wellness is a House call and telehealth medical practice that provides competent medical care in the patient’s homes as well as virtually. We are specialists in primary care for the elderly as well as chronic care homebound patients. We provide transitional care visits for those patients recently discharged from a hospital or facility such as a rehab or nursing home. This service allows patients to have a medical provider care for them in their homes until they are stable enough to continue outpatient visits with their primary care provider. Patients who are stable and require follow-up for a chronic condition can have virtual/telehealth visits. These visits can also be used for simple problems visits or our medical marijuana visits. Our certified providers can provide prescriptions and certifications for medical marijuana to eligible patients in New York State.
